THE ELBOW HEALTH STACK

The tool used in the original Tyler Twist research. The rubber bar allows simultaneous grip loading and forearm rotation, producing a more sport-specific eccentric stimulus than a dumbbell alone. Instruction videos are freely available from TheraBand. The red (light) bar is appropriate for most adults beginning the protocol; progress to green (medium) as the tendon responds. Cost: approximately twenty dollars.

A counterforce brace — a firm band worn on the forearm about 2 inches below the elbow — reduces the load on the lateral epicondyle during activity by dissipating force across the forearm. It does not treat the underlying tendon problem, but it significantly reduces pain during play and allows continued participation during the treatment period. Wear during play only, not continuously.

Racket Equipment Adjustments

Three equipment changes that independently reduce lateral elbow load in racket sport players:

  • String tension: reduce string tension by 5 to 10 percent from current level. Higher string tension transmits more vibration to the elbow at ball contact.

  • Grip size: grip circumference should be such that the index finger of the non-gripping hand fits snugly in the space between the fingertips and the palm of the gripping hand. Too small a grip requires excessive muscle tension to control the racket.

  • Racket weight and balance: a heavier, more head-light racket absorbs more impact than a light, head-heavy racket. If symptoms are severe, borrowing a heavier racket for a trial period is informative.

  • Today — The elbow health toolkit: FlexBar, counterforce brace, and three racket equipment adjustments. String tension, grip size, and racket weight and balance.

Tennis elbow is one of the most successfully treated overuse injuries when the right intervention is used. The right intervention is eccentric loading. Everything else is pain management.
