THURSDAY DEEP DIVE: THE STRENGTH MOVE
Sunday’s issue introduced Tandem Stance. Today we add perturbation training — the most specific available training for fall prevention — and a reactive stepping drill.

PROGRESSION 1: PROGRESSIVE TANDEM STANCE (BEGINNER THROUGH ADVANCED)
The clinical tandem stance progression used in fall prevention programs:
Level 1: Feet together, eyes open — hold 30 seconds. (Most people can do this easily.)
Level 2: Tandem stance (heel to toe), eyes open — hold 30 seconds. (Sunday’s exercise. Falls below 30 seconds indicates fall risk.)
Level 3: Single-leg stance, eyes open — hold 30 seconds each side.
Level 4: Tandem stance, eyes closed — hold 10 seconds. (Very challenging. Few people over 65 can do this without training.)
Level 5: Single-leg stance, eyes closed — hold 10 seconds each side.
Determine your current level and train at that level daily until 30 seconds is reliable, then advance. The progression from Level 2 to Level 4 represents a transformation in fall risk profile.

PROGRESSION 2: PERTURBATION TRAINING
The most specific training for actual fall prevention: being gently pushed or pulled during a balance task trains the neuromuscular response to unexpected perturbation — which is what a real fall involves.
How to do it (requires a partner):
Stand in tandem stance or single-leg stance
Have a partner apply gentle, unpredictable pushes to your shoulders, hips, or sternum from different directions
The goal is not to fall — it is to practice the rapid, automatic balance correction that prevents falling
Start with very light touches and increase the challenge as the response improves
2 to 3 minutes of perturbation per session, 2 to 3 times per week
Perturbation training is the only balance training that directly practices the actual motor program that prevents a fall in real life. All other balance training builds the sensory and strength foundation for this response; perturbation training trains the response itself.

PROGRESSION 3: REACTIVE STEPPING DRILL
Best for: Training the stepping response — the rapid protective step that the body takes to recover from a near-fall — which slows significantly with age and is the primary determinant of whether a loss of balance becomes an actual fall.
How to do it:
Stand in a comfortable stance, eyes open
Have a partner (or use a stick leaning against your shoulder) create a sudden, unexpected weight shift
Take a rapid step in the direction of the weight shift to recover balance — do not grab a wall
Reset and repeat from different directions
Alternative (solo): stand close to the edge of a step, lean forward until the weight shifts past the balance point, then take a rapid forward step to recover
10 to 15 reactive steps per session, 2 to 3 sessions per week
The stepping response speed — measured from perturbation onset to foot contact — is one of the most important fall-prevention variables and one that most balance programs never specifically train.
